Coaching Requirements

FENWICK HIGH SCHOOL / ARCHDIOCESE REQUIREMENTS: 

There are several compliance requirements that need to be completed for the Archdiocese of Chicago’s Safe Environment Program. Please access compliance information online at www.virtus.org and follow the attached directions for completing an online background check application.  

One of the compliance requirements is for digital fingerprinting. The Archdiocese only accepts results from Accurate Biometrics (www.accuratebiometrics.com) and there are several locations to choose from. You will need to complete and bring the attached Fingerprinting form with you to an Accurate Biometrics location.

While volunteering for Fenwick High School, you will be expected to adhere to the Archdiocesan Standards of Behavior which is applicable to all employees or volunteers at Fenwick. Please see the Standards of Behavior Acknowledgement form at the Virtus website and electronically sign it.  You will also need to complete two forms for the Illinois Department of Children and Family Services (CANTS form 689 and form 22) which are attached. Please return both CANTS form directly to Fenwick Hockey Registrar and Margo Schmitt (mschmitt@fenwickfriars.com) and she will forward to DCFS.

Every employee/volunteer is required to attend the Virtus Protecting God’s Children program for adults. You can sign-up for this training at the link on the Virtus website (www.virtus.org). It is currently being conducted online and in-person at a church parish. Please register online for the session you are most comfortable taking. 

Mandated Reporter Training (MRT) is another required training program through the Archdiocese of Chicago. Use this link to access MRT:  https://mr.dcfstraining.org/UserAuth/Login!loginPage.action.  MRT training can be completed online and the last screen of the training session will need to be printed which is the certificate of completion. (You can use the last four digits of your social security number for the access code.)  

If you have already attended one or both of the training sessions, please forward copies of your certificate(s) to Fenwick Hockey Registrar and Margo Schmitt (mschmitt@fenwickfriars.com).

Please forward all compliance paperwork (Code of Conduct Acknowledgement form, CANTS form, Virtus training certificate and the Mandated Reporter training certificate) to Fenwick Hockey Registrar and Margo Schmitt (mschimitt@fenwickfriars.com) at your earliest convenience.
